The following are several forms that are used for City Business. For your convenience, follow the links and print the forms to fill out prior to coming to City Hall. Forms need to be submitted with payment and full documentation and/or a clear copy of ID where indicated. Permit applications submitted by businesses with prepaid accounts can be faxed, however, please do not fax any application that requires a copy of your driver's license, as they do not come through the fax clearly.
