Start New Service

To start utilities / trash services, please visit City Hall and complete a new Utility Contract (PDF), pay the deposit, and provide a driver license.  


To ensure accurate service, when you are preparing to move out or sell your home, please click this link to complete a Request to Disconnect form, which can also be found in the right hand column under Related Documents, or you may come by City Hall and complete the form in person.  

Additional Carts

If you need to add any additional polycarts to your trash service, please visit City Hall and pay the deposit for the additional cart(s). Each additional cart requires a deposit of $15.00 at time of request.  This can be done at any time you determine you need more carts.  

You should already have one black-lidded cart and one yellow-lidded cart, unless your service address is a new build.  

Applying by Mail

You can print the Utility Contract, fill it out, and return it to us by mail with your payment if you are unable to come in during business hours. Our phone number and fax number are at the top of the Utility Contract. Please call to verify your deposit amount required before mailing it.

Additional Information

For issues with your Water, Sewer, and Trash bill, please contact City of Choctaw Utility Billing at 405-390-8198.  

All bills are due on the 15th of every month.  Accounts that are not paid by the 15th will incur a $10.00 late fee.  These accounts will also receive a late notice, auto-generated by our billing software.  If you pay after the 15th, but before the mailing of the late notice, you may still receive a late/cut-off notice.  You may call and verify your account status at any time.   Please note that online payments take 24 hours to post, and the most current information may not be available if you have recently made an online payment.
